For many men, the hardest part of deciding on gynecomastia surgery is not the surgery. It is the thought of being put under. The good news is that gynecomastia surgery under local anesthesia, often called “awake” gynecomastia surgery, is a real option for many patients. This guide explains how local and general anesthesia compare, who tends to suit each one, and how the choice is made at VIA Health & Medspa in Chandler. It is written for men across the East Valley, including Gilbert, Mesa, Tempe and Scottsdale.
The three options in plain language
Local anesthesia (awake)
Numbing medicine is placed into the chest so the treatment area cannot feel pain. You stay awake and can talk to the team. You may feel pressure or movement, but not sharp pain. VIA’s awake gynecomastia approach uses local anesthesia only.
Local anesthesia with sedation
The chest is numbed the same way, and you also get medication to help you relax. You are calmer and may remember less, but you are not fully unconscious the way you are under general anesthesia.
General anesthesia
You are fully asleep for the whole procedure, and your breathing and vital signs are managed and monitored throughout. This is the traditional route for larger or more involved cases.
At VIA, gynecomastia surgery can be done under local anesthesia with sedation or under general anesthesia, depending on the extent of the procedure and your preference. Dr. Wissam Fayad talks through the best option for your comfort and safety at your consultation.
What happens during awake gynecomastia surgery?
Whatever the anesthesia, the surgery itself follows the same steps:
Numbing. The chest is numbed thoroughly before any incision is made.
Liposuction for fat. Through small incisions, a thin tube called a cannula removes excess fat and shapes the chest.
Removing the gland. Firm glandular tissue that liposuction cannot remove is taken out through a small incision, usually along the edge of the areola.
Contouring and closure. The chest is shaped for a natural, masculine look, the incisions are closed (often with dissolvable stitches), and a compression garment goes on.
At VIA, awake gynecomastia can also be paired with Quantum Ignite RF, a radiofrequency skin-tightening treatment, for men who would benefit from some extra skin tightening.

Local vs general anesthesia: side by side
Local (awake) | General | |
|---|---|---|
Are you asleep? | No. You are awake and numb. | Yes, fully asleep. |
Grogginess afterward | Usually little, since there is no general anesthetic to wear off. | Common for some hours. Some people feel nauseous. |
Best suited to | Many small-to-moderate cases with fat and gland. | Larger or more involved cases, or patients who prefer to be asleep. |
Cost | Often lower, because there is less anesthesia and facility overhead. | Usually higher, with an anesthesia fee and facility fee added. |
Getting home | Ask the team. Plan for a ride, especially with sedation or medication. | You will need someone to drive you and stay with you. |
Who is a good fit for awake surgery?
Awake surgery often suits men who:
Have a mix of fat and gland that can be treated through small incisions
Are in good general health, with no unmanaged medical conditions
Feel comfortable staying awake, or would actively prefer not to be put under
Want a quicker return to daily life
Have realistic expectations and are non-smokers, or are willing to stop for a period before and after surgery
When general anesthesia makes more sense
A large amount of tissue, or a more involved case that takes longer
Significant loose skin that needs more extensive work
Strong anxiety about being awake, even with sedation

Neither option is “better” across the board. The right one is the one that keeps you safe and comfortable and gets the result you want.
Does it hurt if you are awake?
This is the number-one question. The numbing is what makes the surgery comfortable. Most men say they feel pressure, tugging or vibration rather than pain. The most noticeable moment for many people is the initial numbing. If you are nervous, sedation is there to help you relax.
How anesthesia affects recovery
Your chest heals the same way whichever anesthesia you have. You will still have swelling, bruising and a compression garment. What awake surgery can change is the first day: no waiting for general anesthesia to wear off. VIA describes its awake approach as offering minimal downtime and a fast recovery. Most patients return to light activity within a few days to a week, and strenuous exercise waits about three to four weeks. How anesthesia affects cost
General anesthesia usually adds an anesthesia provider’s fee and an operating-room fee, which is a big reason quotes around Phoenix vary so much. Awake surgery avoids much of that overhead. See our gynecomastia surgery cost guide for Arizona for typical price ranges and what to ask when you compare quotes.
Questions to ask at your consultation
Is my chest fullness mostly fat, mostly gland, or both?
Am I a good candidate for awake surgery, or would you recommend sedation or general anesthesia, and why?
What will I feel during the procedure, and how is discomfort managed afterward?
Would skin tightening such as Quantum Ignite RF help in my case?
How much time off work should I plan for, given my job?
What does my quote include, and are financing options available?
Frequently asked questions
Can you do gyno surgery without anesthesia?
No. Gynecomastia surgery always uses anesthesia of some kind. “Awake” means local anesthesia numbs the chest while you stay conscious. It does not mean surgery without pain control.
How risky is twilight sedation?
Every form of anesthesia carries some risk, and your health history decides what is appropriate. Dr. Fayad reviews your medical history and medications at your consultation and recommends the safest option for you.
What does gynecomastia surgery involve?
It removes excess fat with liposuction and firm glandular tissue through a small incision near the areola, then shapes the chest and closes the incisions. A compression garment supports healing afterward.
Is gynecomastia surgery under local anesthesia permanent?
Yes. The anesthesia does not change the result. Removed glandular tissue and fat cells do not grow back, though significant weight gain or an untreated hormonal cause can affect the chest later.
Will I see or feel the surgery while awake?
The surgical area is kept out of your view, and the numbing blocks pain. Many men simply chat with the team or relax.
