One of the big reasons people choose scarless eyelid surgery is the recovery. There are no stitches on the skin and no outer scar to heal, so many patients find it easier than they expected. It is still surgery, though, and knowing what each stage looks like helps you plan work, family and social plans around it. Here is what recovery usually looks like, from the drive home to the final result.
A quick refresher: what was done
Scarless eyelid surgery (transconjunctival lower blepharoplasty) removes or repositions the fat that causes under-eye bags. The surgeon works through a small incision on the inside of the lower eyelid, so the outer skin is not cut. At VIA Health & Medspa in Chandler, it is typically done under local anesthesia with sedation and takes about one to two hours. The inner incision is closed with dissolving stitches or left to seal on its own. More detail is on our scarless eyelid surgery page.
Because the opening is inside the lid, recovery is mostly about swelling and bruising settling, not about a skin wound healing.
Recovery timeline at a glance
When | What is common | What to focus on |
|---|---|---|
Procedure day | Mild swelling, some bruising, drowsiness from sedation | Someone drives you home; cold compresses; rest |
Days 1–3 | Swelling at its most noticeable, mild discomfort | Head elevated, cold compresses, over-the-counter pain relief |
Days 4–7 | Swelling and bruising start to fade | Gentle activity is usually fine; no rubbing your eyes |
Weeks 1–2 | Most bruising and swelling resolve | Still no heavy lifting or hard workouts; follow-up visit |
Weeks 2–4 | Initial result becomes visible | Ease back into normal routines as cleared |
Months 2–6 | Tissues keep settling | Final, refined result appears gradually |
Everyone heals at their own pace. Use this as a general guide and follow the instructions your care team gives you.
Procedure day
When the procedure is done, you may notice mild swelling or bruising around the eyes. Cold compresses are often used right away to keep that down. Since you will have had sedation, you need someone to drive you home. Plan to spend the rest of the day resting.
Tip: set up a recovery spot before surgery day: extra pillows to keep your head up, cold compresses ready, and anything you need within reach.
Days 1 to 3: rest and keep your head up
The first few days are when swelling is usually most noticeable. Two things help most:
Keep your head elevated, including when you sleep.
Use cold compresses as instructed.
Most discomfort can be managed with over-the-counter pain relievers. Ask your care team which ones are right for you, since some medicines can increase bruising. Avoid anything strenuous, and do not rub your eyes, even if they feel dry or itchy.
Days 4 to 7: turning the corner
By the end of the first week, most people notice swelling and bruising beginning to fade. Gentle activities, such as short walks around the neighborhood or light tasks at home, are typically fine after a few days. Many patients who work at a desk feel ready to return to work somewhere in this window, but it depends on how you feel and on what your care team advises. If you would rather nobody notices, some patients plan a full week off.
Weeks 1 to 2: most bruising clears
Bruising and swelling after scarless eyelid surgery are usually mild and resolve within one to two weeks. During this time, keep avoiding:
Heavy lifting
Vigorous exercise, such as running, HIIT classes or heavy weights
Anything that raises your blood pressure, like straining or bending over for long periods
VIA recommends avoiding these for at least one to two weeks. You will also have a follow-up appointment so the team can check your healing and answer questions.
Weeks 2 to 4: seeing your early result
Once most of the swelling and bruising has gone, usually within a few weeks, you will start to see your initial result: a smoother, flatter under-eye area and a more rested look. If one side seems a little puffier than the other, that is common early on, as swelling does not always fade evenly.
Months 2 to 6: the final result
The final, refined outcome becomes clearer over several months as the tissues fully settle. Try not to judge your result too early. For most people, the improvement from scarless eyelid surgery is long-lasting, often ten years or more, although genetics, lifestyle and normal aging all play a part.

Recovering in the Arizona sun
Healing in Chandler and the rest of the East Valley comes with a few local considerations:
Wear sunglasses outdoors. They protect healing eyelids from bright sun and wind, and hide any bruising on errands.
Watch the dry air. Desert air can make eyes feel dry. Ask your care team what eye drops or ointment you may use, and avoid rubbing.
Stay out of the heat for workouts. When you are cleared to exercise, start with light activity indoors or in the cooler morning hours rather than a hard hike in the afternoon heat.
Avoid dusty outdoor work early on, such as yard work on a windy day.
Signs to call the office
Mild swelling, bruising and some tenderness are normal. Call the office if something feels wrong, for example pain that gets worse instead of better, swelling on one side that keeps increasing, changes in your vision, or any sign of infection. When in doubt, call. That is what the team is there for.
How to have a smoother recovery
Follow your pre-op instructions on diet, medicines and substances to avoid.
Arrange your ride home and a few days of help if you have small children.
Keep your head elevated and use cold compresses in the first days.
Do not rub your eyes.
Hold off on heavy lifting and hard exercise for at least one to two weeks.
Keep every follow-up appointment.
Frequently asked questions
How long after scarless eyelid surgery will I look normal?
Most people look presentable within one to two weeks, once bruising and swelling settle. The initial result shows within a few weeks, and the final result over several months.
Is recovery easier than traditional eyelid surgery?
Because there is no outer skin incision and no stitches on the face, many patients find recovery more comfortable. You still need to rest and follow activity limits. Your surgeon can explain how the two approaches differ for your eyelids at your consultation.
Is bad bruising normal after eyelid surgery?
Some bruising is common and is usually mild after the scarless technique. Bruising that keeps getting worse, or comes with increasing pain or vision changes, is a reason to call the office.
When can I exercise again?
Avoid heavy lifting and vigorous exercise for at least one to two weeks. Gentle walking is usually fine after a few days. Your care team will tell you when to return to full workouts.
Will I have stitches to remove?
No outer stitches are used. The inner incision is closed with dissolving stitches or seals on its own.
